What is Advance Care Planning?

Advance care planning involves preparation around care you wish to receive should you be too ill or unable to make those decisions when care is needed. Care planning plays a large role in chronic disease management and palliative care where often doctors and family members are making decisions on the patient’s behalf.

Without advance care planning this can result in difficulty for friends and family members trying to predict what you would should you be unable to speak for yourself.

These choices are formally made as an Advanced care directive which is a legal document which goes into effect if you are otherwise unable to speak for yourself due to a disease or injury.
